Tuesday Telehealth Tip: Your no-show rate isn't a scheduling problem (here's what it actually is)

Okay. Let's talk about something that quietly drains more revenue from practices than almost anything else.

No-shows. Telehealth has made it easier to meet with providers, but sadly it has shown to also make it easier to blow them off.

I hear some version of the same thing from nearly every provider I work with: "We send reminders. We have a cancellation policy. I don't know why people just disappear."

And I want to offer a different frame today — because if you think your no-show problem is a scheduling problem, you'll keep solving the wrong thing.


The Real Reason Patients Don't Show Up

When I dig into no-show patterns with practices, it almost never comes down to forgetfulness or bad intentions. It usually comes down to one of three things:

  • Friction at the point of access. The patient got a reminder, clicked the link, hit a tech snag or a confusing login, and quietly gave up. Nobody called it a cancellation. It just became a no-show.
  • Emotional hesitation they never voiced. Especially in telehealth and behavioral health — patients second-guess themselves between booking and the appointment. If there's no touchpoint to catch that hesitation, you lose them silently.
  • A reminder that reminded them, but didn't re-engage them. "Your appointment is tomorrow at 2pm" is a notification. It's not a reason to show up. There's a difference.

None of those are scheduling problems. They're communication and workflow problems. And the fix looks completely different.


What Actually Moves the Needle

You don't need a new scheduling platform. Here's what works:

  • Replace confirmation texts with micro-engagements. Instead of "Confirm your appointment," try "You've got a virtual visit tomorrow — here's what to have ready." It reduces friction AND re-establishes the value of showing up.
  • Add one human touchpoint for high-risk appointments. New patients, long gaps since last visit, behavioral health — these warrant a brief personal outreach, not just an automated ping. 5 minutes from your front desk can save a $200-$400 missed slot.
  • Audit your join experience. Have someone on your team attempt to join a visit as a patient would — cold, on a phone, without help. You'll find the friction point in under three minutes.
  • Set a 48-hour re-engagement trigger. If a patient hasn't confirmed 48 hours out, that's your window to catch hesitation before it becomes a ghost. Most EHRs and patient engagement tools can automate this.

Small changes. Real data. Build from there.


The Cost of Doing Nothing

A practice seeing 20 patients a day with even a 10% no-show rate is losing 2 slots daily. At $150 average visit value, that's $300 a day — over $75,000 a year — quietly walking out the door.
