Tuesday Telehealth Tip: AI Powered Remote Patient Monitoring

Hi Reader

Welcome to Week 3 of our AI in Telehealth series! Last week we tackled AI scribes and documentation assistants—tools that can give you back hours of your week by handling the charting you used to do at 9 PM.

Hope you had a chance to talk to a colleague about their documentation after last week's challenge.

Today we're looking at something that extends your care beyond the visit itself: AI-powered remote patient monitoring (RPM).

What Is It?

RPM uses connected devices—think blood pressure cuffs, glucose monitors, pulse oximeters, smart scales—to collect patient data between visits. More and more, even consumer wearables like your Apple Watch, Oura Ring or Whoop Band are being used in clinical settings. AI takes it a step further by analyzing that data in real time, spotting trends, and alerting your team when something needs attention. Instead of waiting for a patient to tell you something's wrong at their next appointment, you can catch it early.

Why Should You Care?

  • Catches warning signs days or weeks before they become ER visits or Psychiatric Emergencies
  • Especially powerful for chronic conditions like CHF, diabetes, COPD, and hypertension and Medication Adherence
  • AI filters the noise so your team only sees alerts that actually matter—no more drowning in data
  • Patients feel more connected to their care team between visits
  • RPM has its own set of billing codes (99453–99458)—this isn't just better care, it's a revenue opportunity
  • Pairs naturally with your Telehealth visits to create a more complete care model

A Few Things to Keep in Mind

  • Alert fatigue is real—work with your vendor to set smart thresholds so your staff isn't chasing false alarms
  • Patient compliance matters—if they don't wear or use the device, the data doesn't flow. Choose devices that are simple and intuitive
  • Know the billing requirements—CMS requires data transmission for at least 16 days in a 30-day period to bill RPM codes
  • As always, HIPAA compliance is non-negotiable—make sure devices and platforms are transmitting data securely
  • Not every patient is a good candidate—start with your highest-risk patients where monitoring will have the biggest impact

I recognize that a significant portion of this audience is in the Mental Health Space. There are a lot of ways RPM can help your Psychiatric or Therapy clients. We've helped practices figure out which RPM solutions make sense for their specific patient populations and how to actually make it work efficiently. If you're curious, we're happy to talk it through.

This Week's Challenge:

Look at your patient panel and identify 3–5 patients with chronic conditions or medication needs that might benefit from remote monitoring. Then take 10 minutes to explore one RPM platform to see what's available. Let me know what you find!

When you can see what's happening with your patients between visits, and they begin to take control of their own improvement your treatment becomes preventative, not just reactionary.

Coming Up:

Next week we'll wrap up the series with AI-powered patient engagement and communication—how to use AI to keep patients informed, connected, and on track without adding more to your plate.

FREE WEBINAR ALERT: I'm hosting a live webinar the first week of March where we'll dive deeper into all of this! Registration is LIVE!

Hi Reader -Dan Mountain Last week I asked you to think about Chronic Care Management and Revenue Cycle Management. Many responses involved learning how added collections can help your practice and many of you are interested in learning how to get more of the revenue back that you have earned. Today we are starting a 3 week series on Billing and Reimbursement because denials are becoming more and more of an issue for Telehealth Providers. Let's face it, Payers are using AI to deny more often,...

Hey Reader, Here's something I see constantly with small mental health and wellness practices: They set up Telehealth during COVID, it worked "well enough," and now it's just... there. A way to keep patients happy. What it's not being used as is a revenue tool. That's a missed opportunity — and it's more common than you'd think. When your virtual care setup is actually dialed in, a few things happen: You stop losing patients to friction. A clunky intake process, a platform that confuses...

Hi Reader What's taking up too much of your time? Last week I asked you to write down the one task you do manually, repeatedly, and kind of dread. A lot of you replied. And I want you to know — I read every single one. The answers fell into two buckets, administrative work and keeping patients engaged. Not your clinical work, It's the administrative grind that follows. That's not a coincidence. And it's not a personal failing. It's a systems problem — and there are tools built specifically to...
