Tuesday Telehealth Tip: AI Powered Remote Patient Monitoring

Welcome to Week 3 of our AI in Telehealth series! Last week we tackled AI scribes and documentation assistants—tools that can give you back hours of your week by handling the charting you used to do at 9 PM.

Today we're looking at something that extends your care beyond the visit itself: AI-powered remote patient monitoring (RPM).

What Is It?

RPM uses connected devices—think blood pressure cuffs, glucose monitors, pulse oximeters, smart scales—to collect patient data between visits. More and more, even consumer wearables like your Apple Watch, Oura Ring or Whoop Band are being used in clinical settings. AI takes it a step further by analyzing that data in real time, spotting trends, and alerting your team when something needs attention. Instead of waiting for a patient to tell you something's wrong at their next appointment, you can catch it early.

Why Should You Care?

  • Catches warning signs days or weeks before they become ER visits or Psychiatric Emergencies
  • Especially powerful for chronic conditions like CHF, diabetes, COPD, and hypertension and Medication Adherence
  • AI filters the noise so your team only sees alerts that actually matter—no more drowning in data
  • Patients feel more connected to their care team between visits
  • RPM has its own set of billing codes (99453–99458)—this isn't just better care, it's a revenue opportunity
  • Pairs naturally with your Telehealth visits to create a more complete care model

A Few Things to Keep in Mind

  • Alert fatigue is real—work with your vendor to set smart thresholds so your staff isn't chasing false alarms
  • Patient compliance matters—if they don't wear or use the device, the data doesn't flow. Choose devices that are simple and intuitive
  • Know the billing requirements—CMS requires data transmission for at least 16 days in a 30-day period to bill RPM codes
  • As always, HIPAA compliance is non-negotiable—make sure devices and platforms are transmitting data securely
  • Not every patient is a good candidate—start with your highest-risk patients where monitoring will have the biggest impact

This Week's Challenge:

Look at your patient panel and identify 3–5 patients with chronic conditions or medication needs that might benefit from remote monitoring. Then take 10 minutes to explore one RPM platform to see what's available. Let me know what you find!

When you can see what's happening with your patients between visits, and they begin to take control of their own improvement your treatment becomes preventative, not just reactionary.
